Birth Certificate and Social Security Card?

My girlfriend and I have been having a very difficult time trying to obtain our daughter's Social Security Card and Birth Certificate we applied for one at the Hospital and have never received it (Although it shows on the records that a SSN was registered to her, as I got the number for filing Taxes) We have called our local office even drove nearly 100 something miles to just get the paper with her number on it not even the actual card. I have filled out different forms mailing them in only to get them back saying I can't request that information because I wasn't on the Birth Certificate even though at the hospital (Tucson Medical Center) when we filled out the request it had my information on it as well as the mothers. Any information helps and one last thing this is all for Cochise County, AZ. She was born in Pima County (Tucson, AZ) At TMC
